Early Head Start Substitute Teacher
SUMMARY:
To provide a successful, safe and supervised educational setting with children with stable and supportive relationships including developing strong, consistent relationships while they are in the Early Head Start environment. Promote the social, emotional, physical, and cognitive development of Early Head Start children. Provide frequent contact with the families of EHS in the classroom and at home. To keep accurate written notes in relation to developing goals for children, while providing on-going assessments and facilitate successful transition into and out of the program.
Family Advocate-Family Empowerment Center (FEC)
SUMMARY:
Under the supervision of the FEC Program Coordinator, the Family Advocate provides support, outreach, and training to families of children and young adults between the ages of 3-22 with special needs. The Family Advocate will provide comprehensive and culturally competent support services to clients. Educate families on resources based on their current needs and assist in the development of community support systems and networks. Provide client advocacy to families with all disabilities. Assist parents with understanding their child’s education and developmental needs, communicate with service providers and serve as a resource and support through the IEP process. The Family Advocate works with families to build upon strengths and to enhance self-sufficiency. Provide support, mentoring, resources, referral, education, training, activities, and assistance to the family to identify and utilize resources within the community.
Licensed Clinical Social Worker (LCSW)/SUN Navigator
SUMMARY:
The Navigator shall provide substance use screenings, advocate for follow-up individual/group treatment, therapy, discharge and aftercare planning to patients and their families. The Navigator is one of the primary contacts for involving and educating patients, families, and significant others in the process of substance use and mental health treatment and recovery. They shall also serve as a liaison to community programs and develop strategic partnerships to enhance aftercare plans of patients and their families.
